To submit the Declaration of Reservation, please complete the Form for Receipt of Item with Reservation 'Declaration of Reservation' from the list of subjects above, including your reasons. The timely submission of the Reservation Statement is required for the later submission of an application for compensation. ELTA accepts no responsibility for the item's total or partial damage, or partial loss/theft, if the recipient does not indicate reservation of receipt within one (1) working day of delivery.
